Pack Supermarket at 8235 NE 2nd Ave in Miami was ordered shut for the third time after health inspectors found a dead rat and multiple rodent violations during a complaint-based inspection. The Miami establishment was closed on March 20 with 21 violations found, marking its third shutdown in less than six months after previous closures on December 8, 2025, and October 7, 2025.

Inspectors documented extensive rodent activity throughout the facility, finding droppings in critical food preparation areas. “Observed approximately 3 rodent dropping on service card with unused cooking equipment at kitchen area. Also observed approximately 5 rodent droppings next to single service utensils and cups on top of counter at kitchen area,” according to the inspection report. The facility also had roach infestations, with inspectors noting “5 live roaches inside empty unused walk in cooler” and finding “1 dead roach on the floor under shelf at walk in cooler.”

The violations extended beyond pest control issues, with inspectors finding tomato sauce being held at unsafe temperatures and standing water on floors throughout the facility. Pack Supermarket was among several South Florida restaurants cited for health violations last week, including establishments in Wilton Manors, Aventura, Fort Lauderdale, and Cooper City.

Other restaurants faced similar pest-related closures across South Florida during the same inspection period. Georgie’s Alibi Monkey Bar in Wilton Manors and Azumare in Aventura were also cited for rodent issues, while Takato in Fort Lauderdale had dozens of small flying insects landing on clean plates and sushi mats. Restaurant Brands International in Miami was found to have mold-like buildup inside an ice machine used for drinks.

At Origen in Cooper City, inspectors documented cross-contamination violations when an employee handled raw chicken and then went straight to ready-to-eat food without washing their hands. The facility also had grease caked behind equipment and ceiling tiles with damage and holes hanging over prep areas, creating additional health hazards.

All restaurants except Origen in Cooper City have been allowed to reopen after completing required cleanup procedures and passing re-inspection. Pack Supermarket’s repeated violations and third shutdown in six months highlight ongoing compliance issues at the Miami location, which must now undergo additional scrutiny before reopening to the public.
